Lab Rotation Notice / Evaluation Form Advisor Record Form Change Advisor Form Leave Sheet If you will plan to leave Taiwan (no matter what reason/purpose), you are required to fill out the Leave Sheet and obtain approval with signatures from the thesis advisor and program coordinator before the scheduled leave. The signed form is required to be handed in to the office to complete the application.
